Yes - a Home Medicines Review (HMR) is a structured, GP-referred service where an accredited pharmacist reviews your medicines in your own home.

Can a pharmacist review my medicines at home?

Home Medicines Reviews

A comprehensive review of all the medicines and vitamins you’re taking, conducted in your home by an accredited pharmacist, with findings reported back to your GP.

It’s a comprehensive medicine check-up in the comfort of your home, and can help catch interactions or issues your GP may not otherwise see.

What is a Home Medicines Review?

This service may be suitable if you:

  • Take multiple long-term medicines and would benefit from a structured review
  • Have recently been discharged from hospital
  • Have been identified by your GP as at risk of medicine-related problems

Who May Benefit from a Home Medicines Review?

  • Requires a GP referral.
  • Frequency limits and eligibility criteria are set by the HMR program.

Any changes to your medicines are made by your GP, not by the reviewing pharmacist directly.

Important Information

Before Your Visit

Be prepared to discuss:

  • All the medicines you’re currently taking, including over-the-counter and complementary products
  • Any concerns you have about your medicines
  • Relevant medical history where applicable

During Your Visit

An accredited pharmacist will visit your home and go through all the medicines you’re taking.

The review may also include advice on medicine timing, storage, or potential interactions, with a written report sent to your GP.

What to Expect During Your Visit

Do I need a referral?

Yes, a Home Medicines Review requires a referral from your GP.

Can I request a Home Medicines Review myself, or does it have to come from my GP?

The referral has to come from your GP, though you’re welcome to raise it with them if you think you’d benefit.

What happens after the review?

The pharmacist’s report goes to your GP, who will discuss any recommended changes at a follow-up appointment.
